Below are various forms that TEAM-Science Teacher Leaders might find useful.

Substitute Reimbursement Form (Right click here to download form in Word format)

Teachers who attend the TEAM Science Teacher Leader workshops during a normal contract day can have their school reimbursed for the Substitute's pay.  Print off this form fill it out and have it signed by your principal.  Then bring the filled out form to the TEAM Science Teacher Leader workshop.
